The following are required by Sharjah Municiplaity Clinic upon processing your request:

  • Current passport copy

  • Valid visa copy

  • One photograph (not more than 6 months old)

  • Trade license copy (if applicable)

  • Passport copy of sponsor with copy of valid visa (if applicable)

  • Copy of Health Card issued by Ministry of Health or Health Insurance attested by the Ministry of Health

  • Copy of Emirates Identity Card or stamped copy of the typed form for Emirates ID

Terms & Conditions:

  • The client will be responsible for all details entered on the e-application; any changes in details will make the application void
  • If the client is working in an occupation categorized by the Municipality to annually renew the Municipality Occupational Health Card, this service cannot be presently used
