- Certificate of approval for sanitization materials (Chemicals) from an accredited laboratory (Only companies practicing sterilization and disinfection activities)
- Detailed procedures used in sanitization by the company (Only companies practicing sterilization and disinfection activities)
- Details of persons in charge of sterilization and disinfection operations (Scientific Qualification) (Only companies practicing sterilization and disinfection activities)
- Material Safety Data Sheet (MSDS) of used chemicals (Factories if any, Buildings cleaning companies, companies practicing sterilization and disinfection activities)
- Attach the previous building cleaning permit (Building Cleaning Companies only)
- In case the required procedures are not completed the transaction will be canceled after 30 days from application date.
- Attach last environmental permit in case of renewal or change trade license.
Important / After submitting an application, please visit our reception ( Environmental Protection Section ) in Industrial Area 5 (Customer Service Center) to provide a record of inspection and follow-up.
